Personal Data Processing Consent

I, the undersigned data subject, hereby consent to the processing of my personal data by Individual Entrepreneur Ekaterina Mikhailovna Medzhidova (Taxpayer ID (INN) 091705972928, State Registration No. (OGRNIP) 325774600418811, address: Apt. 202, 19 building 2, Rozhdestvenskaya Street, Moscow), hereinafter the “Operator”.

I give this consent freely, of my own will and in my own interests.

1. Personal data covered by this consent

1.1. Surname and first name.

1.2. Contact telephone number.

1.3. Email address.

1.4. Preferences for the location and date of the event (concert).

2. Purposes of processing

2.1. Considering a concert booking request.

2.2. Contacting me to clarify the request (location, date and event arrangements).

2.3. Entering into and performing a concert agreement, if an agreement is reached.

3. Operations performed on personal data

3.1. Collection, recording, organisation, accumulation, storage, clarification (updating and modification), retrieval, use, depersonalisation, blocking, deletion and destruction.

3.2. Both automated and non-automated methods are used (mixed processing).

4. Duration of consent

4.1. This consent remains valid until the processing purposes have been achieved, but for no longer than three (3) years from the date it is given, unless the laws of the Russian Federation provide otherwise.

5. Withdrawal of consent

5.1. I may withdraw this consent at any time by sending written notice to the Operator at: Apt. 202, 19 building 2, Rozhdestvenskaya Street, Moscow, or to isentie.music@gmail.com.

5.2. Upon receipt of withdrawal, the Operator stops processing my personal data and destroys it within thirty (30) days, except where processing may continue under the laws of the Russian Federation.

6. Data subject’s acknowledgements

6.1. I confirm that my rights under Federal Law No. 152-FZ of 27 July 2006 “On Personal Data” have been explained to me.

6.2. I confirm that I have read the Operator’s Personal Data Processing Policy and that the Operator provided its text before I gave this consent.

6.3. I understand that personal data processing consent cannot be included in the Terms of Use or any other document that I sign or accept together with this consent.

7. Method of giving consent

7.1. This consent is given by ticking the checkbox next to the text “I consent to the processing of my personal data in accordance with the Personal Data Processing Consent” when submitting the concert booking request form on the ISENTIE website.

7.2. The date of consent is the date on which the request form is submitted.
